Protection and regeneration of the inner ear

The lab of the group ’Protection and Regeneration of the Inner Ear’ of the department Otorhinolaryngology of the Hannover Medical School is located in the NIFE. A close interrelation between clinicians and scientists within this group aims to develop novel therapeutic strategies for the inner ear that result in rapid translation to the clinic. The main research focuses on cell-based approaches to deliver proteins and drugs over extended time periods to the inner ear during cochlear implantation.

Research

Hearing loss is one of the most prevalent neurodegenerative disorders affecting one in six Europeans (Hear it.org 2019). Despite extensive investment in medical research, only a handful of new therapeutic products emerge each year. To date there is no class of inner ear drugs available for preventing inner ear damage or for treating hearing loss and associated conditions.

Restoration of hearing is based on technical devices either with hearing aids for sound amplification or with cochlear implants for direct electrical stimulation of the auditory neurons. Although cochlear implants restore sound perception in many patients, cochlear implantation is not sufficient for speech understanding in a large number of patients emphasizing the need for improvement. Cochlear implant electrode arrays are inserted into the inner ear, providing an invaluable route to access the otherwise inaccessible cochlea. Both diagnostic sampling of inner ear fluids (perilymph) and local intracochlear application of therapeutics are therewith possible.

The combination of basic research and clinical research within our group aims at developing novel cell-based therapeutic strategies for the inner ear that result in a rapid translation to the clinic. There are multiple approaches for addressing inner ear disease that will change the future of hearing impaired people and especially cochlear implant users. Analyzing cochlear perilymph samples characterizes the molecular profile of cochlear implant patients and aids to identify novel therapeutic targets. Cell-based approaches show a variety of advantages including the possibility to deliver complex proteins and drugs over extended time periods.

A key factor in the development of cell therapy is the source of the cells. Concentrating on autologous (proprioceptive) cells and cell products, protection and regeneration of the inner ear is envisaged and may enhance the performance that can be achieved with a cochlear implant. Currently, biohybrid electrodes (ROEMER ET AL. 2016), hat have been developed in our research group are implanted routinely in our clinic to reduce foreign body reaction and inflammation. Human mesenchymal stromal cells (MSC) derived from bone marrow are utilized as autologous cell source for the cell based systems. These cells release immunomodulative factors to balance inflammation and have a highly protective effect on cochlear cells.

We hypothesize that the therapeutic effects of MSC are mainly related to their secretion of biologically active factors and specifically to extracellular vesicles (EVs) rather than to their differentiation properties. The nano-sized EVs are released by nearly all cell types and contain highly active biomolecules from their parental cells: proteins, different nucleic acids, carbohydrates, lipids and metabolites. By exhibiting low immunogenicity and toxicity, a long half-life time and the ability to cross the blood-brain barrier EVs present a promising approach to deliver a plethora of various factors acting as therapeutic agents. With regard to future clinical application we investigated human umbilical cord-derived MSC-EVs showing an increased survival of cochlear neurons in vitro and protection of hair cells and hearing from noise trauma in vivo (WARNECKE ET AL. 2020).

In this context, genetic modification as well as structuring of the implant surface will be performed in order to induce and control differentiation of MSC. In addition, the endogenous regenerative capacities of the inner ear will be explored in cochlear-specific in vitro cell culture systems including microsurgical inner ear preparation techniques and different in vivo models (noise trauma, fibrosis, residual hearing). Stem cell technology, molecular biology, electro physiological measurements as well as modern imaging technologies are combined in an interdisciplinary approach to pursue our envisaged goals.
